EMR (Electronic Medical Record) Funds Applicant Portal

The Rural Health Transformation Program (RHTP) is a multi-year, CMS-approved initiative to strengthen health care across rural Maine. These Year 1 EMR funds help providers modernize the systems that hold patient records - replacing aging platforms, upgrading existing ones, and adding capabilities such as population health, data reporting, AI tools, telehealth, coding and billing, connectivity, and cybersecurity - so care is better connected and measured by outcomes rather than paperwork.

This portal is for Maine-based healthcare organizations only. All eligible organizations have received application directions.

Organizations should submit only one application.We recommend only one authorized representative from each organization create an account.
